Important Insurance Information

Please review the following policies before booking your appointment:

• Medicaid: We are sorry, but our office does not accept Medicaid or any insurance plans managed by Medicaid. We apologize for any inconvenience.

• HMO Plans: If you have an HMO insurance plan, you are required to obtain a referral from your Primary Care Physician (PCP) before your appointment. If a referral is not on file for your visit, you will be responsible for the full payment at the time of service.
